Shack DISH Network $100 Rebate Card Promotion: In order to qualify for the $100 Rebate Card, you must be a new subscriber who has ordered DISH Network America’s Top 120 with Local Channels (& higher) through SatelliteBuys.com, a RadioShack.com-affiliated site. Redemption form must be postmarked no later than 60 days after service activation. Remember to fill out all required fields of the form. The signature must be the account holder’s. Required documents include: a printed out and signed Redemption Form and a copy of the front and back of your first month’s billing statement. Be sure to make 2 copies of each; enclose 1 completed and signed Redemption Form along with the copy of your first month’s bill, and mail them to the address on the form. Keep the other copies for your records. Certain restrictions apply. Offer void where prohibited, taxed or restricted by law. Offer redemption rights cannot be transferred.
